Training in Neurology: Implementation and Evaluation of an Objective Structured Clinical-Examination Tool for Neurology Postgraduate Trainees in Lusaka, Zambia

Background We established Zambia's first neurology residency program at the University of Zambia School Medicine and Teaching Hospital in Lusaka. Objective To evaluate feasibility effectiveness a modified objective structured clinical examination (OSCE) to assess skills. Methods The training program's 3 participants completed OSCE exercise February 2019. used smartphones videotape trainees performing physical oral presentation clinic. Trainees faculty reviewed videos independently using standardized rubric then met for in-person feedback. Results Three pre- post-OSCE surveys rating their confidence elements history examination. Trainees' average self-confidence scores improved from survey every category (pre-OSCE: mean score 6.84, range 4.8–7.8, SD 0.92; post-OSCE: 7.9, 5.67–9.33, 0.86). Qualitative feedback showed found helpful, routinely applied feedback, would appreciate repeating OSCEs. Conclusions OSCEs improve trainees' can be successfully implemented resource-limited postgraduate program. Important modifications involved videotaping real patient encounter rather than patient. Embedding experience within busy clinic day was practical, applicable, efficient. Future work should expand use both Zambian non-neurology programs. Including additional video reviewers could add validity skills assessment. Videos also remote mentorship teaching purposes.
